Initial Assessment + Bioidentical Hormone Replacement Therapy Orientation

We start with a comprehensive medical history and physical exam. In this head to toe assessment, we’ll address any ongoing problems or symptoms you may have, we’ll review whether these might be related to hormone imbalances, and we’ll begin to put a plan of action in place.

During this appointment, Dr. Matthew will discuss how hormonal imbalances may be affecting your health, or may be the root cause of your medical issues. We’ll review Bioidentical Hormone Replacement Therapy, and you’ll learn what benefits you might expect from this therapy based on your symptoms.  Lab tests will be ordered based on your personal history and health goals.

Along with recommending a treatment plan, the doctor will help you decide which of our Signature Wellness Total Health programs best suits your needs, based on your medical issues and the level of staff interaction and support you want.

The Optimal Wellness Program is for people who have struggled unsuccessfully to lose weight.  It’s for people with chronic medical issues that, with proper treatment, could be controlled, such as high blood pressure, fibromyalgia, and high cholesterol. It’s for people who simply don’t feel like themselves anymore due to low libido, low energy levels, or a myriad of symptoms that arise when the body is not functioning optimally.

This unique, 6 month program is designed specifically for people ready to make big changes, but who want the support and guidance of a team of experts to make it happen. The Optimal Wellness Program is more than just doctor visits; it’s an approach to managing your health that puts an entire team of medical experts in your corner.


How it Works


We start with an in-depth analysis of where you are today. Based on your medical history and your goals, you’ll undergo a series of sophisticated and comprehensive tests to assess your current hormonal balance, your nutritional status, your stress levels and your risk for chronic disease.

Lab Evaluation 


Based on your symptoms we may check:

• Sex hormones(estrogen, progesterone, testosterone)

• Stress hormones (cortisol, DHEA)

• Metabolism (blood sugar metabolism and hunger regulation hormones)

• Thyroid hormones

* Complete blood count

• Kidney and liver function

• Vitamin D levels

As needed, we may also conduct the following innovative tests:

• Comprehensive digestive analysis

• Food sensitivity testing

• Exposure to heavy metals

• Evaluation of nutritional status

• Body composition analysis (body fat/muscle mass)

We’ll also put together a pro-active plan for your health by evaluating a range of bio-medical

markers that may predict future health risks. This may include tests such as:

• Genomic testing for inherited risk factors

* Inflammatory markers

* Advanced cardiovascular risk factors

* Estrogen metabolism for cancer risk

• Urine bone breakdown markers indicating osteoporosis risk

Nutrition Assessment


We’ll conduct a complete nutritional assessment and prepare a dietary plan. This is done to help meet weight loss goals (when appropriate), but also to address medical issues affected by your diet and digestion. Conditions like joint pain, mood swings, chronic digestive symptoms, high blood sugar, high cholesterol and high blood pressure can all be impacted by the food you eat, and how it’s digested.


Your Plan

Once all of the analysis is complete, we’ll compile a personalized wellness program to meet your goals and address any current health issues and symptoms. This includes our medical weight loss program or specialized nutrition program, hormone pellet therapy where appropriate, stress management counseling, and a tailored regimen of pharmaceutical grade vitamins and supplements. Then together, we’ll get to work.


After reviewing your personalized plan, you’ll work together with your team. During this time, you’ll have regular access to your team including visits with the doctor, weekly check ups with your nutrition counselor and access, as needed, to your patient educator. (Check ups and monitoring can take place in the office, on the phone or via email, based on your needs.) This ongoing monitoring helps us to address more issues simultaneously and adapt your program as needed, so that you can see results more quickly.

Labs will be repeated to assess your progress as needed.  At the conclusion of your program we will work with you to decide on an ongoing course of treatment to keep you on your path to a healthier you.

The Executive Wellness Program is for patients who are living a healthy lifestyle today. You eat right and you exercise, but you know you could be doing more. Whether that’s Bioidentical Hormone Replacement Therapy to better regulate mood and metabolism or gaining an understanding of where you may be at risk in the future. This 4 month program is designed for those patients who are working hard today to ensure they will live a healthy tomorrow.

How it Works


After gaining a complete understanding of your medical history, and what you’re doing today to stay fit, we’ll begin looking for areas where we can improve your already healthy lifestyle. We’ll take a focused look at how your body is performing, and search for any areas that can be improved.

Your Plan


After tests are complete and your baseline is established, you’ll have an extended appointment to discuss a personalized plan to optimize your health. Depending on your goals, this may include Bioidentical Hormone Replacement Therapy and a new pharmaceutical vitamin and supplement treatment tailored to your specific goals. You’ll receive a customized health binder that includes your test results and your personalized wellness plan.

During your program, you’ll check in monthly with your team (by phone or in person). At the end of the program lab tests will be repeated. You’ll have another in-depth appointment during which  your progress will be discussed and an updated course of treatment will be decided upon.



Designed to get you back in balance.

The Hormonal Wellness Program is for people experiencing the classic symptoms of hormonal imbalance; fatigue, night sweats, menstrual problems, insomnia, low libido, anxiety, irritability or depression.  This  program focuses exclusively on getting your hormones back in balance.

How it Works


You’ll start with an appointment to review the results of the labs ordered during your Initial

Assessment. We’ll decide which of your symptoms are due to hormonal imbalances and determine a course of action to treat the problem.

Lab Evaluation


Depending on your symptoms, we’ll review test results such as:

• Sex hormone levels (estrogen, progesterone, testosterone)

• Stress hormones (cortisol, DHEA)

• Complete thyroid evaluation

• Vitamin D levels

Your Plan


One month after starting your new hormone therapy, you’ll check in with your nurse to ensure that you’re progressing and your symptoms are subsiding.  An additional round of tests will ordered during your third month so we can be certain you are making improvements.

At the four month mark, you’ll sit down with your doctor for a comprehensive review of your progress and to determine what changes, if any, need to be made to your treatment. During this appointment a plan will be made to continue to monitor your progress moving forward.
